Please note: All entries and fees must be submitted via the Clubs New Zealand online portal. Once you have completed this form, simply pass it on to your Club, who will complete the online form on your behalf.

* A $2 administration fee applies to each online booking.

$360 per team ($90 per player)

*PLEASE NOTE: this fee does not include the prizegiving dinner which is $40pp and is optional - booking and payment will be taken at registration.
